Identity: why the name matters more than the card

Clearance: the background check

Health: what the clinical facility needs

The 40 clinical hours happen in a licensed nursing facility, and the facility's infection-control policy is the source of every health requirement. The usual set is a TB screen dated within the last 12 months, documented MMR and varicella immunity, hepatitis B vaccination or a signed declination, a Tdap within 10 years, and a seasonal flu shot during flu season. Records can come from a clinic printout, a school immunization record, a prior employer's occupational health file, or the Washington Immunization Information System through a provider.
